Title 21 CFR Part 11 is the part of Title 21 of the Code of Federal Regulations that establishes the United States Food and Drug Administration (FDA) regulations on electronic records and electronic signatures (ERES). Part 11, as it is commonly called, defines the criteria under which electronic records and electronic signatures are considered trustworthy, reliable, and equivalent to paper records (Title 21 CFR Part 11 Section 11.1 (a)).
With QuestionPro verified signature question type, the authencity of the respondent is verified by confirming a 4 digit code sent to the respondents email address. Once signed electronically, a response is considered reliable, and equivalent to paper records. A copy of the response is also emailed to the respondent.
To add a verified signature question, go to: Login » Surveys » Edit
Click on Add Question button, and select the Advanced question types menu.
Click on Verified Signature question to add.
Verified signature question will be added to your survey. You can send a PDF copy of the response to respondents by enabling the toggle for Respondent Notification under Settings
Respondents will be asked to enter their full name and email address. A verification code will be sent to the email address for verification.
Respondents have to enter the verfication code.
Once verfied, respondents can type in the signature in the box and also check the agreement check box.
