The position of acting first officer was bestowed upon an officer or another individual aboard a starship when the ship's regular first officer was unable to fulfill their duties or until a permanent replacement had been found.
In an alternate 2258, Captain Christopher Pike of the USS Enterprise appointed Commander Spock acting captain and Lieutenant James T. Kirk as first officer when Pike left the ship to confront Nero. Subsequently, Kirk proved that Spock was unable to perform his duties and then Kirk himself was made acting captain. After everything was over, Kirk was promoted to the rank of captain and given regular command of the ship. (Star Trek)
In 2259, Lieutenant La'an Noonien-Singh was made the acting first officer of the USS Enterprise on two occasions: after its regular first officer, Una Chin-Riley, went missing on Kiley 279 (SNW: "Strange New Worlds"), and again after Una was injured in a Gorn attack on the Enterprise. Lieutenant Commander Data was made the acting first officer after the disappearance of Captain Jean-Luc Picard in 2370. (TNG: "Gambit, Part I")
Lieutenant Tom Paris filled the role of acting first officer after Captain Kathryn Janeway was assimilated by the Borg in 2377. (VOY: "Unimatrix Zero, Part II")
In 2384, Dal R'El served as Captain Chakotay's acting first officer on the USS Protostar after Dal and his young crew found the ship marooned on Ysida. At one point, Dal briefly took command of the ship while Chakotay prepared himself for their rendezvous with the USS Voyager-A. Chakotay later had Gwyn serve as his acting first officer after Dal joined Nova Squadron when Voyager and the Protostar faced off against the Rev-1. (PRO: "Ascension, Part I", "Ascension, Part II")
When the young former crew of the Protostar were assigned to the new USS Prodigy, Dal decided to serve as acting first officer while Gwyn was the acting captain instead of being the acting captain himself like Dal had done on the Protostar. (PRO: "Ouroboros, Part II")
In 2401, When Captain William T. Riker was made acting captain of the USS Titan-A, Admiral Jean-Luc Picard served as acting first officer under Riker. (PIC: "Seventeen Seconds")
Ensign Sylvia Tilly filled the role of acting first officer of the USS Discovery after Commander Michael Burnham was demoted from the position for insubordination in 3189. (DIS: "Unification III")
In 3191, Commander Rayner had now-Lieutenant junior grade Tilly take up the role of acting first officer once again while Captain Burnham was aboard Ruhn's dreadnaught. (DIS: "Lagrange Point", "Life, Itself")
