These crewmen were sent on an away mission by then-commander Kathryn Janeway to survey a volcanic moon. Three were injured due to a magma eruption. Guilt-ridden, she went alone to finish the mission the day afterward to assure them they had not been hurt in vain.
In 2375, Tuvok recounted this story to Chakotay as an example of Janeway's tendency to blame herself for such occurrences. (VOY: "Night")
