after you install mod engine for Sekiro, just download the version you want of this mod, create a mods folder in your game directory and inside create a Sounds and put the main.fsb you downloaded there
thank you for quick response it seems like the mod for the deathblows is broken though for me at least I tried it a few times and nothing i downloaded the one with just perilous attacks and it works fine
this is weird, I just tested here and it worked just fine I just downloaded, extracted it and put it in the Sound folder inside my mods folde in my game directory and it worked, seems odd that this didn't work for you

maybe there was a problem with the download or the game version, make sure your game version is up to date to the 1.06 and try re-downloading the mod and extracting it from WinRAR, i just tested here and it worked perfectly, good luck to you man!!
First you need to download the main file: main.fsb (which any mod or sound modification has here at Nexusmod) Second you gotta open the DDSI. Click Open, select the main.fsb, Put the audio quality to a 100% Name your sound file to the name of the sound you wanna replace: For example: Perilous Attack is s000600035 so your audio file should be named s000600035 to replace it in DDSI. Click Add Sample and select your audio file, it should be saying that the target is set already if you putted the same audio file name as the one you wanna replace. Now click in Execute Insertion. If it says "Untested windows version detected", don't click anything or the program will crash, just wait for it and the Insertion will be completed. After all that just put the main.fsb file you edited in the Sounds folder of your mods folder and you should be good to go.
