As we wrap up our Horizon 2030 Care at Home case study series, we've explored innovative programs from leading health systems like Mayo Clinic, Vanderbilt, Johns Hopkins, Mass General Brigham, and Advocate Health. These programs are pioneering the future of healthcare by delivering high-quality care directly to patients' homes, optimizing value, and improving patient outcomes. Key Thematic Commonalities Across Leading Health Systems → 1: Process: Successful Care at Home programs are strategically designed to meet specific health system and community objectives. → 2: Infrastructure: Virtual command centers and care solutions significantly improve patient access to healthcare, effectively complementing in-person care. Remote patient monitoring (RPM) is tailored to patient conditions and risk levels, enhancing the effectiveness of care delivery. → 3: People: Implementing measured change management strategies and providing education are crucial for encouraging provider adoption of Care at Home models.
