I feel humbled by the rich experiences in both my personal & professional life which has taught me earlier on in life to view failures as stepping stones for growth & development. I agree with Amy Edmondson that learning from failure isn't easy. As I often tell those who I coach & mentor, take the "shame & guilt out & make place & room for humility & courage. Through courage & humility we pave the path for acceptance. Through acceptance, we open the doors for accountability. With accountability we start paving the path for clarity & transparency & there one shall find the errors & mistakes as well as solutions, in most beautiful of ways & options." Change your mindset & start from there", as my loving siblings say. To err is human & what you decide to do with it & from that point on, think of Einstein where he said, to achieve a different result, one must also change the approach & make the effort!

Learning from failure is hard work and requires a humility, introspection and courage.

Powerful and simple framework to learn from failure: 1. Examine what happened. 2. What did I do to contribute to the failure? Or fail to do? (Error of Omission).
