Great to connect with and learn from leading innovative health systems and solution providers at the Hospital @ Home Leadership Summit. It's clear that the movement to home-based care for patient needs across the care continuum is real, is much needed, and is accelerating, and those who aren't considering the home as a part of their overall care delivery approach will fall behind. Here are a few of the themes that my Chartis colleagues Mark Krivopal, MD, MBA and Ryan Bertram found particularly notable: *There’s an growing distinction between 2 segments of health systems: those who are ‘H@H-curious’, are planning programs, or have just launched, and those with established programs. The former are looking to better understand how to activate their program. The latter are looking to better understand how to scale and expand. * There’s a growing movement to evolve from the singular H@H use case to more of a holistic care delivery platform that can support the ecosystem of care across the continuum. *Culture, Culture, Culture. Regardless of the stage of program maturity, health systems find cultural challenges inherent in the innovative care model represented by H@H to be the largest impediment to activation and growth. Comprehensive and persistent communication and education programs are absolutely required for success. #culture *While there’s general optimism about an increasingly favorable regulatory and reimbursement environment at both the federal and state levels, there are still open questions about how to navigate that landscape.
