Last week I listened to an interview with Forward founder and CEO Adrian Aoun. The part of the interview that stuck with me was the concept that healthcare was built and operated as a service, and significant gains could be achieved if healthcare was delivered as a product. He argues that service businesses are hard to scale because they're reliant on scaling people -- doctors and other caregivers in the case of healthcare. This creates a pretty hard ceiling to scaling as you expand access to more consumers. For example, if we solved the problem of everyone having the ability to afford care, and motivated everyone to actively engage in their health, there simply wouldn't be enough physicians to meet the demand. At least not in our current sick-care system. By contrast, if healthcare was a product primarily delivered through software, devices, and other scalable assets that were imbued with the vast clinical insights of these same doctors and caregivers, this ceiling could be significantly raised and significant improvements in access and utilization could be gained as a result. This is the promise of digital health because scalability is one of its key advantages. I know what you're thinking. That seems a bit simplistic. Perhaps. However, after setting that initial thought aside, the concept resonates because of the potential gains if applied appropriately. The key is "applied appropriately". The virtual care vs. in-person care debate is usually an either/or discussion. Not only is that unproductive, it also holds us back from the next chapter of a more integrated and accessible system. Allow me a quick segue to connect a thread to my post from last week about a healthcare system vs. a sick care system. What if this healthcare-as-product approach was focused on preventative medicine and lower acuity conditions? In this scenario, the scarce resources of physicians could be reserved for those who need additional care and intervention. I can envision other benefits: 1. Increased innovation in the management of those conditions that drive high utilization (especially chronic conditions) but could be well managed by standard treatment and management protocols. 2. There could be significant health equity gains because tools like GenAI could be trained on a larger, more heterogeneous pool of data to identify risk factors for specific populations. 3. Clinicians would theoretically have additional resources to operate at the top of their license by focusing on the more challenging cases. 4. There would be significant improvements in patient and provider experience and therefore willingness to participate. Naturally, there would need to be connectivity between these two paths. As we've learned, silos are not productive when it comes to improving health outcomes and efficiency gains. There would also need to be on/off-ramps based on patient needs as their health status evolves. Thought-provoking. #digitalhealth #healthcare #integratedcare
