This article sheds more light on the #healthcare access challenge we’re facing in the U.S. Expanding access to care is one of our highest priorities at Sutter Health so we can help more patients lead better, healthier lives and make it quicker for them to receive care. This includes adding clinical capacity by growing our workforce – including the number of #physicians affiliated with our system. We’re not only recruiting today’s physicians, we’re investing in tomorrow’s through a significant commitment to and expansion of our Graduate Medical Education programs, which will ultimately train 1,000 residents and fellows a year. We’re also expanding our presence in the communities we serve by building new care centers, enhancing our existing facilities to ensure they best meet the needs of those we serve, and using #digital tools to make our care more accessible. It’s a big challenge and there’s much more to be done, but we’re making real progress thanks to great work by teams across our organization and our focus on making it easier for patients to navigate their care and engage with us when, where and how they want!

San Francisco achieved one of the lowest Covid-19 death rates and had one of the highest vaccination and booster rates, compared with rates across California or the United States.  This is thanks to the “San Francisco Health Systems Collaborative” which formed during the Covid-19 pandemic which now serves as a national model in pandemic response.   Zuckerberg San Francisco General stood at the forefront of the pandemic response. Our doctors, nurses and administrators witnessed and experienced surges in cases and hospitalization, pushing the limits of our hospital every day. But through medicine, research, and collaboration with San Francisco Public Health and UCSF Health, and other local hospitals, health partners, and the community we all unknowingly became leaders – simply by doing what we do best: providing equitable care to the community by protecting vulnerable people, protecting health care workers, and maintaining health system capacity.   While another pandemic in the future is inevitable (hopefully not again in our lifetime), I feel grateful and honored to have contributed to the planning and preparations for the health of future generations.   Read more in the New England Journal of Medicine: Catalyst article https://lnkd.in/gsS2ucwj (https://lnkd.in/gsS2ucwj)

A recent CHCF survey reveals stark disparities in health care access for Central Valley residents. The recent symposium in Fresno brought together community leaders, health advocates, and experts to address the findings: • 56% believe their communities lack sufficient mental health providers • 48% carry medical debt (vs. 36% statewide) • 35% report increasing difficulty accessing affordable, quality care • 50% face unreasonable wait times for physical health appointments • 78% have a regular doctor (vs. 85% statewide) These findings underscore the urgent need for targeted solutions to improve health care access and affordability in the Central Valley. The Arizona Department of Health Services has recently undergone a significant overhaul of its licensing structure, aiming to enhance the regulation and oversight of behavioral health and assisted living facilities. This restructuring has resulted in the establishment of two specialized entities: the Bureau of Behavioral Health Facilities Licensing (BBHL) and the Bureau of Assisted Living Facilities Licensing (BALF). By consolidating regulatory responsibilities and offering more targeted oversight, these bureaus seek to ensure the safety and well-being of patients and residents. The Department of Health Care Access and Information (HCAI) recently held a Health Care Affordability Board (HCAB) meeting to outline a 10-year primary care spending plan with the Office of Health Care Affordability (OHCA). This initiative is designed to significantly enhance primary care spending in California, which is expected to lead to increased access to whole-person, comprehensive, coordinated primary care and a reduction in hospital and emergency department visits. Elevating Chronic Care Management through Nurse Care Managers As healthcare continues to pivot towards value-based care models, the role of #NurseCareManagers (#NCMs) in Chronic Care Management (#CCM) has never been more pivotal. NCMs are at the forefront of identifying eligible patients, developing personalized #careplans, and ensuring seamless #carecoordination. But how does this translate into better Quality Payment Program (#QPP) scores and increased incentive payments? First, by identifying patients eligible for CCM, NCMs ensure that those with multiple #chronicconditions receive continuous, comprehensive care. They not only enroll patients but also provide them with the necessary education about the benefits of such programs, leading to higher patient engagement and satisfaction. Secondly, care coordination is the heart of CCM. NCMs work tirelessly to connect the dots between various healthcare providers, ensuring that the patient's care plan is executed flawlessly. This includes managing medication, scheduling follow-ups, and monitoring the patient's health status. The seamless execution of these plans leads to improved patient outcomes and reduced hospital readmissions, which are critical metrics in determining QPP scores. Lastly, meticulous documentation and correct coding of CCM services by NCMs ensure that providers are reimbursed accurately for the care provided. This not only supports the financial health of the practice but also reflects the quality of care in QPP scores. In essence, NCMs are invaluable in navigating the complexities of chronic care, enhancing patient health outcomes, and steering healthcare practices towards higher QPP scores and increased incentives.
