    Partners in Care Foundation is pleased to welcome Dan Swayze, DrPH, MBA, MEMS of Pittsburgh, PA, as our new Chief Operating Officer (COO) We are excited about Dan joining our organization. He brings a wealth of experience gained from over thirty years of health care/social care alignment management with vulnerable populations and patients in both home and community-based settings. Prior to joining Partners, he served as Vice President of Community Services at UPMC Health Plan in Pittsburgh, PA, where he has led a team of more than one hundred multidisciplinary, in-home care managers. Dan has been the recipient of numerous awards, including the Lifetime Achievement Award from the Academy of International Mobile Healthcare Integration (2024) and the Excellence Award from the Pittsburgh Emergency Medicine Foundation (2023). Dan earned a doctorate in Public Health and two Masters Degrees (MBA and Masters of Emergency Medical Services). Swayze is married to a wife of thirty years, and together they have raised one son and one daughter. Welcome to the team Dan!

    Partners' is delighted to introduce Marti Ferrara, M.S. who is joining our team as our Senior Director of Marketing Communications. With over 15 years of experience in the healthcare industry, including the last eight years at Elevance Health leading transformative initiatives in Medicare and Medicaid business strategy and marketing, Marti brings a unique blend of strategic marketing expertise and a deep-seated passion for ensuring healthcare access for all. In this role, Marti will plan, implement, and manage strategies to enhance our branding, positioning, and growth. She will oversee marketing communications, conduct research and analysis, and manage the development and budget of marketing programs. Marti will also lead efforts in program and service promotions, community relations, and strategic communications. Outside of work, Marti loves spending time with her family. She and her husband, Paul, recently celebrated their 21st anniversary. They have two wonderful and talented daughters, Elena and Kailin, both in college.     Meet Nick. Nick lives with diabetes, a history of prostate cancer, and has a history of falls. Living alone with no support system is challenging and he experiences limitations in his ability to perform basic daily tasks. Nick enrolled in Partners Multipurpose Senior Services Program (MSSP), and they immediately helped connect him with tools and resources like In Home Supportive Services (IHSS), CalFresh (food stamps), and a Rollator Walker. The MSSP Care Manager helped Nick apply for IHSS hours and he now receives help several times a month with household chores, cooking and cleaning, tasks that once were a burden to him. To better manage his diabetes, he receives support with his meals and groceries through the CalFresh program. CalFresh provides food stamps to assist low-income individuals, like Nick, in purchasing nutritional food items to meet his needs. MSSP was also able to connect him with a medical supply store, via Mercy Plaza Respiratory, that provided Nick with a rollator walker to help with his balance and stability and reduce overall falls. Thanks to the assistance of MSSP services, Nick can continue to live safely alone and comfortably in his own home.
