Don't focus on who - name, blame, shame isn't going to help !! The biggest thing is to understand what went wrong and why so you can address how to fix the problem now and avoid repeating the issue in future. After action review, post incident review, debrief, lessons learned - conducting an analysis of the situation to develop a strategy to move forward is essential. Get the right people in the room to have the discussion, make the necessary decisions, and agree next steps, then develop a new plan for progress and any corrective actions to be taken. Things go wrong, it happens all the time. The key thing is how you respond to it and what you do next - identify the problem, work out the solution, plan next steps and move forward.
