Packaging for me was great, game came in perfect condition. And the game itself is a must play if you're into a bit of Thriller and exploring and enjoy learning good lore. Story is fantastic and encourages you to explore to learn more outside of missions and cutscenes. Do not sleep on this game! It's not often you get games this good and unique these days in the AAA market.
The game is designed to be challenging though, so there can be areas that will overwhelm you. However, the game has Assist Mode to make your life easier, and you can literally have the option to ONE SHOT enemies if you need it. And no, it will not deactivate Achievements/Trophies.
