Running into similar issue when upgrading from 2.4 to 4.0
Failed to perform query "ALTER TABLEwp_redirection_itemsADDmatch_urlVARCHAR(2000) NULL DEFAULT NULL AFTERurl"
There is a beta version that should fix this issue here:

If you want to wait I will be releasing it via the usual channels soon. Your redirects will continue to work.
We’ve tried the beta plugin but keep getting stuck on the “Upgrade Database” page with a Something went wrong error.
Is there a good way to nuke the Redirection data and start fresh from the backup we created?
What is the something went wrong error? That is different to the original problem you had

Hi, I’m also trying to sort out the Upgrade Database fail issue. I downloaded the 4.0.1 beta. When I try to upload the zip file to a site via “Upload Plugin”, it also fails. Any ideas here? It seems to have worked for the previous commenter, so hopefully it’ll work for us, too, if we can get our Redirection upgraded to the beta version.
Hi i have upgrade database… and the home page is direct to an old article… and i can’t find the old article redirect to delete it … what i mast do now ?

How can activate 4.0.1 beta 3 to fix the problem … if i delete the plug in i will lost oll the redirect ( i have 300 redirect)
@kratisionline, sorry I don’t really understand. What is redirecting to a 404 page? 4.0.1 beta 3 is not the latest version. Why will you lose 300 redirects?

Yes but I have never made this change myself. and there is nowhere to delete it.
What can i do now ?
