The blacksmith in the little village near Bald Mountain appears at the start of the Bald Mountain quest. Before that, only foglets can be seen at the location, and they return after the quest completes as well, but he will remain there regardless.
He doesn't sell anything interesting, having the inventory and pricing of generic Velen blacksmiths, but can be useful for repairs and getting rid of excess gear, as Bald Mountain is part of a long sequence of main quests during which fast traveling is disabled and gameplay is restricted to story-relevant areas.
