Game Ending bug, Manor Key

I’m about 60 hours in on the ps4, and after looking for the desert manor key for hours I realized death is supposed to give it to you, but he didn’t I checked my stash and all characters inventories. I finished and place the fire purgatory stone before the other 3 were replaced could that be why? Is there any way to get another one of these keys?

I had to restart I forget exactly what I did, but I my second Play through I made sure to do all the levels in the correct order I think this issue happens if your break the intended sequence

Anyone else run into this? I killed the Fire boss before the Black Fortress boss and can't get the manor key - any way to trigger it besides restarting? Or a way around the door?
