For me, this issue seems to occur most commonly when watching videos in 4k or 1440p. I've had it occur on both Windows and Linux (using nvidia-vaapi-driver for hardware acceleration).

 It can manifest in any of the following ways:
- The video freezes for a few seconds, and will freeze consistently in the same place when seeking back, even if I seek into the middle of the frozen portion of the video.
- The playback position instantaneously jumps forward by a few seconds. This seems to often be accompanied by the buffered portion of the timeline flickering between what seems to be the correct buffer duration and a longer or shorter duration while it plays back.
- Most rarely, the video will just get stuck buffering permanently.
It's worth noting that I have the MSE video buffer size set to 150MiB, per Bug 1760529. This may help to prevent that permanent buffering issue from occurring, but I'm unsure if that's the case.

I only started having these issues pretty recently, but by running mozregression on Windows, I've confirmed that I have issues as far back as July of last year. I wonder if something could have changed on YouTube's side to cause these issues to appear so suddenly?
